Stifel raises HeartFlow stock price target to $45 on plaque growth

Stifel lifted its HeartFlow (HTFL) price target to $45 from $40 and kept a Buy rating after the company’s Q2 2026 results beat estimates. HeartFlow posted Q2 revenue of $64.1M vs $56.7M projected (and $56.6M consensus) with core U.S. FFRCT revenue up ~32% and gross margins rising to 83.3% (quarterly high); adjusted EPS loss narrowed to -$0.07 vs -$0.19 expected. The firm also raised FY2026 revenue guidance to $246M–$250M from $228M–$232M, supporting a bullish read-through as shares gained ~50% over six months and rose after-hours post-release.

Analysis

This is a classic “good quarter, dangerous multiple” setup. The market will likely reward the revenue beat immediately, but the more important variable is not the print itself; it is whether reimbursement converts into sustained site-level utilization. If the new plaque category is still only a low-teens mix, the stock is already discounting a much steeper adoption curve than is typical for a nascent diagnostic platform, so any hiccup in ordering cadence or payer normalization can compress the premium quickly.

The second-order winners are the CT workflow ecosystem and upstream imaging vendors that benefit from more coronary CTA throughput, while the obvious losers are lower-yield legacy stress-testing pathways and some invasive diagnostic volume. That said, hospitals may resist faster adoption if the tool reduces cath-lab conversion rates on borderline cases; the clinical win can be a near-term economic loss for facilities, which slows penetration despite reimbursement support. In other words, a better gatekeeper can paradoxically create its own adoption friction.

Time horizon matters: over the next few days momentum can carry the stock higher, but over 1-3 months the key catalyst is whether reimbursement translates into repeatable utilization and guide-up cadence. Over 6-18 months, the bull case is a durable AI diagnostics platform with expanding gross margin leverage; the bear case is an over-earning growth story that normalizes as easy comps fade. Falsifiers are simple: any slowdown in gross margin expansion, plaque mix, or guidance lift would argue the rerating has outrun the underlying demand curve.
