BeyondSpring Inc. Reports Promising Interim Phase 2 Data for Plinabulin in Metastatic NSCLC Showing 6.8 Months Median PFS and 78% Overall Survival Rate

BeyondSpring (BYSI) announced interim Phase 2 results for Plinabulin combined with pembrolizumab and docetaxel in metastatic non-small cell lung cancer (NSCLC) patients who progressed after PD-1/L1 therapies, demonstrating a median progression-free survival of 6.8 months and a disease control rate of 77.3%, exceeding current standards. The 15-month overall survival rate was 78%, also promising compared to standard of care, and the treatment was generally well tolerated, suggesting Plinabulin's potential to overcome resistance to checkpoint inhibitors; however, 51.1% of patients experienced grade 3 or higher adverse effects.

Analysis

BeyondSpring Inc. (BYSI) presented promising interim Phase 2 data for Plinabulin in combination with pembrolizumab and docetaxel for metastatic non-small cell lung cancer (NSCLC) patients who have progressed after PD-1/L1 therapies. The study, involving 47 patients, demonstrated a median progression-free survival (PFS) of 6.8 months, significantly outpacing the 3.7 months standard of care (SOC) with docetaxel. Furthermore, the disease control rate (DCR) was 77.3%, and the 15-month overall survival (OS) rate stood at a notable 78%, compared to a median OS of 11.8 months for SOC. These results suggest Plinabulin's potential in addressing acquired resistance to checkpoint inhibitors, a critical unmet need for over 60% of NSCLC patients. While the combination therapy was generally well-tolerated with no treatment-related deaths, 51.1% of patients experienced grade 3 or higher treatment-related adverse effects. The confirmed objective response rate (ORR) was 18.2%, higher than the 12.8% for SOC docetaxel, though it indicates a majority did not achieve an objective response. The small sample size warrants caution in generalizing these findings. Institutional investor activity shows a mixed sentiment, with six firms adding BYSI shares and eight decreasing positions in the recent quarter; notably, Citigroup liquidated its entire holding, while Two Sigma Securities and Citadel Advisors initiated positions. The study, supported by Merck, highlights Plinabulin's dual mechanism of activating GEF-H1 for dendritic cell maturation and T-cell activation, potentially re-sensitizing tumors to immunotherapy.
