Eli Lilly raised full-year guidance to $85B–$87B on Q2 revenue up 48% to $23.0B, driven by $14.9B combined sales growth from Mounjaro and Zepbound. Non-GAAP EPS rose to $8.38 (including $3.03 of acquired IPR&D) and non-GAAP gross margin increased 130 bps to 86.3%, while the company lifted full-year performance margin guidance to 49%–50.5%. Management also launched the Medicare GLP-1 Bridge program covering 20M Americans at $50/month (enabling ~35% more coverage) and expects a Q1 2027 BLA submission for retatrutide after positive Phase 3 results; shares typically benefit from this combination of strong beats plus multiple catalyst timelines.

Analysis

LLY is still the cleanest way to express secular obesity/diabetes penetration, but the important read-through is that the story is shifting from supply-constrained scarcity to managed access. That usually helps unit growth first, then forces a harder look at net price: the quarter implies volume can outrun pricing for now, but the underlying trend is still compression, so upside from here depends on sustained prescription acceleration rather than another rebate-related pop.

Competitive dynamics favor LLY over NVO near term because LLY has both the broader menu and the better execution cadence on access, international rollout, and physician conversion. The second-order winner is the distribution stack around Medicare-adjacent access and retail fulfillment; the losers are compounding/pharmacy leakage and any challenger betting on a purely oral pivot to catch up quickly. Over 6-18 months, the bigger winner may be LLY’s pipeline optionality in OSA, osteoarthritis, and cardiometabolic adjacencies, which could justify a premium if retatrutide filing risk stays contained.

The main risk is that the market extrapolates every access improvement into linear EPS upside while ignoring mix dilution and timing noise. Near term, the stock can keep working if Bridge and ex-U.S. launches show real incremental starts; the thesis breaks if U.S. net price falls faster than volume expands, or if the retatrutide BLA/regulatory path slips into 2027+ uncertainty. Consensus is probably underestimating how much of the current beat is operational and overestimating how much is permanent margin expansion.
