QuidelOrtho reported Q2 total revenue of $630.9M (+1.9% constant-currency), but China revenue fell 23.3% to $67.8M due to uncertainty around proposed IVD pricing guidelines. Adjusted EBITDA rose 21% to $129.3M with margin up 310 bps to 20.0%, yet cash flow deteriorated with operating cash flow of -$111M and free cash flow of -$136M (including a $25M Grifols termination payment and $97M Lex acquisition). The company lowered full-year 2026 revenue guidance to $2.52B–$2.60B (from $2.70B–$2.75B) and adjusted EPS to $0.65–$0.90 (from $1.80–$2.00), withdrawing free-cash-flow guidance amid persistent China headwinds and a softer respiratory season outlook.

Analysis

This is less a revenue story than a capital-structure story. The ex-China franchise is still producing enough operating leverage to keep EBITDA visible, but the equity is now hostage to cash conversion: with leverage already elevated and FCF negative after one-off outlays, every incremental dollar of working-capital drag disproportionately hits the multiple. In diagnostics, that usually means the market starts valuing the business off balance-sheet risk, not gross margin improvement, so near-term valuation compression can continue even if core demand looks stable.

China is the main second-order risk because pricing-guideline uncertainty can contaminate distributor behavior across adjacent imported IVD categories, not just this name. If procurement pauses persist into the next budget cycle, the issue becomes a share-and-channel reset rather than a temporary destock, and that can spill into competitors with China exposure and instrument-heavy models. The respiratory call is also important: a softer season mainly shifts revenue between quarters, but for a levered name it can still force another guidance reset if utilization fails to recover by the winter build.

The contrarian setup is that consensus may be over-anchored to the top-line cut and underestimating the embedded operating leverage outside China. If management proves that inventory unwinds and the Neulixa launch create a cleaner FCF profile by year-end, the stock can re-rate sharply from depressed levels. What would falsify the bearish case is a benign final China pricing rule, a clear respiratory uptick by early fall, or visible working-capital release that restores confidence in 2026 cash generation.
