In ORIGIN 3 interim results at 36 weeks, TRUTAKNA cut 24-hour urine protein-to-creatinine ratio (UPCR) by 46% from baseline and by 42% versus placebo (p<0.0001). Analysis

This is a meaningful de-risking event for VERA, but not a clean “buy-and-forget” reset. The market should price in a higher probability that the asset becomes a durable specialty franchise, yet the commercial ceiling is still gated by diagnosis intensity, payer prior auth, and weekly self-injection persistence; in nephrology, that usually means early uptake concentrates in academic centers before broader adoption follows. The key medium-term metric is not first prescriptions but refill durability and gross-to-net, because the $0 copay messaging can hide a much harsher rebate burden once payers react.

The bigger second-order effect is competitive sequence risk in IgAN rather than outright category takeout. VERA likely pressures the share and pricing power of incumbents that depend on simpler chronic maintenance algorithms, while also making IgAN a more crowded “stack” of supportive care plus disease-modifying therapy; that favors companies with existing nephrology sales infrastructure and may raise the value of diagnostics/monitoring rather than pure biologic innovation. If nephrologists view this as an add-on rather than a replacement, the read-through is less about a monopoly and more about a longer treatment duration market.

The real binary is the Q3 eGFR readout: proteinuria gets the label, but kidney-function data is what supports a sustained re-rating. A neutral or messy eGFR result would likely compress the multiple quickly because today’s optimism is still anchored to a surrogate endpoint and a small, controlled population. Near-term downside catalysts are safety noise, payer friction, or label interpretation that limits use to the sickest patients; upside reverses only if early commercial data show rapid initiation plus low discontinuation.
